Relegating a team

I was thinking about Wolves last game of the season and thought when was the last time we relegated a team in the last game of the season. Alternatively who did we play that the result led to them being officially relegated but still with games to play?

Yep, beat them 2-1 to relegate them on the first weekend of April.
